Please note: if you have ordered directly from the zazen website or by calling our 1300 number and have ordered through one of our team, you will already be in our Customer Database so there is no need to complete this form. You only need to complete this form if you have purchased through a zazen Reseller or you have been given/purchased the system from an existing customer.
